POP WARNER FORMS

All players must have their birth certificate & a recent picture uploaded to their SportsConnect account. (no helmet, sunglasses or hats). You can not be rostered to a team without this information.

​

All players must also print out and turn in at equipment pick up:

1. 2 copies of their 2025-2026 report card

2. A copy of a physical dated in 2026

3. The Player ID card with picture attached. Picture must be current. No helmet, sunglasses or hat permitted.
